As per her mother's wishes, Hazel Grace Lancaster, seventeen years old she reluctantly joins an organization that provides support to cancer patients. To breathe properly, she uses an oxygen tank that she can carry around because of her cancer. She meets a boy at one of her gatherings and learns his name is Augustus Waters. He's there to assist Isaac, their mutual friend. Isaac suffered from a canceroustumor located in his left eye. This was removed successfully. Now , he has to have his opposite eye taken out. Augustus approaches Hazel after the meeting , and tells her that there is a tumor on one eye. He invites Hazel to come to his house and go to the cinema. The two of them also speak about each other's experienceswith the disease. Hazel discovers that she is suffering from thyroid cancer that has expanded to her lung. Augustus had been diagnosed with osteosarcoma and is now cancer-free. Augustus and Hazel decide to share their most loved books before taking Hazel home. Augustus offers Hazel the price of dawn and Hazel suggests an Imperial Fliction.

Hazel writes about the beauty of An Implacable Affliction. The novel is about Anna, a little girl who has cancer. It's the only novel she's ever read that reflects her experiences. She describes how the novel abruptly closes at the end of the sentence which leaves the reader with no closureas as to the fate of the characters. She speculates about Peter Van Houten's mystery author of the novel. She believes has fled Amsterdam shortly after publication. He has not been seen since then.

Augustus magically informs Hazel that he found the assistant of Van Houten Lidewij and was successfully able to establish an email exchange with her. Hazel will share Van Houten's letters with him, and they devise the list of questions to ask Van Houten. The idea is to clarify the mystery's confusing conclusion. Hazel is concerned with Anna's death. She is convinced that Anna's mother will be able to survive the loss of her daughter and her parents will be okay after Hazel's passing. Van Houten finally replies that he's unable to answer questions from Hazel in person. He invites her to visit him if she's ever visiting Amsterdam.

Augustus offers Hazel for a lunch shortly after. He turns out to have planned a Dutch-themed picnic. He reveals that Van Houten is a charity that grants wishes to children with cancer is willing to make his wishes come true. He rubs her face and she is delighted. She realizes she loves him, however, she is certain she will harm him if she dies. She compares herself to a bomb.

Hazel experiences an intense episodeof swelling and fluid buildup in her lungs. She is admitted into the ICU. After a few days Hazel gets released from the hospital and discovers that Augustus has never left the waiting room of the hospital. Hazel receives another letter from Van Houten. The second letter is more personal and obscure. Hazel is now more determined to go to Amsterdam more than ever before after she read the letter. But there's one problem: her parents and team of doctors don't believe she is strong enough to travel. The situation appears impossible till Dr. Maria, one of her most experienced doctors, convincing Hazel that she must travel to an opportunity to live her dreams.

Augustus, Hazel and Hazel's mother are planning to go to Amsterdam. However, When Hazel as well as Augustus first meet Van Houten, they realize that he's not a brilliant genius, but a mean-spirited drunk who can't answer the questions of Hazel. They leave Van Houten's place in complete disbelief and are accompanied by Lidewij, who's shocked by Van Houten's behavior. Augustus and Hazel share a passionate kiss at the conclusion of their tour , to the cheers of the crowd. They return to their hotel, and are reunited with each other for the first time. Augustus confesses to Hazel that he had an MRI scan of his body while within the ICU. It revealed that his cancer was back and spreading all over. Hazel finds out it is Augustus is the grenade, when the two return to Indianapolis. The condition worsensand it is evident that he has lost his charm and confidence. Even though he's more vulnerable and fearful but he's still a handsome boy to Hazel. The moment she stops calling him Augustus and instead calls him Gus like his parents. Hazel discovers that she is in love with him. Augustus's condition deteriorates quickly. Augustus organizes a prefuneral and Isaac and Hazel offer an eulogy. Hazel takes a line from Van Huyten about infinitesimal and greater. She tells Augustus how much she cherishes her and how she would never give up their time together.

Augustus dies eight days later. Hazel is shocked to see Van Houten at her funeral. Van Houten said that Gus and he kept in touch and that Augustus required Van Houten to take a restitution for the excursion in Amsterdam. Van Houten also said his wife Hazel is required to be present at his funeral. Van Houten details Anna's death abstractly, however Hazel doesn't seem to care. Hazel receives a message from Isaac a few days later that Augustus had written something for her. While Hazel tries to find the manuscript van Houten reappears. He is drunk and reveals that Anna was his daughter's name. When she was eight years old, she passed away from cancer. A Imperial Abilities was his literary attempt to find a way to deal with her passing. Van Houten gets a message from Hazel to get sober and write another book.

Hazel is later informed she was Augustus had sent the pages directly to Van Houten as he wanted Van Houten's to write an insightful tribute to Hazel. Lidewij has Van Houten read the book, before transferring them to Hazel. Hazel closes the book by reading Augustus's words. He stated that although it's natural to be wounded in this world, we do have the option of choosing to allow others to take care of it. He was pleased with his choice. He hopes that she will enjoy her decision. Hazel says that she likes what happens at the end of the novel.
